To complete registration with the Penetanguishene Skating Club & Skate Canada, please complete and sign/acknowledge the following forms:

1. Completion of the registration form below
2. Read the required Rowan’s Law Concussion Awareness Resource and The PSC Injury and Return to Sport Reintegration Policy, and complete waiver below.
3. Complete the PSC Media Release waiver below
4. Email forms to infopenetangskatingclub@gmail.com
5. Payment of fees by e-transfer to email above (your PSC registration fee includes your Skate Canada fee).
